    • Medication Management- $60 per visit (60 mins)

      • A comprehensive review of medications, side effects, adherence, interactions, and education. Includes communication with providers as needed.

      • Will fill weekly/biweekly medication container.

    • Wellness Visit- $90 per visit (60 mins)

      • Supportive wellness visit that includes vital signs, status review, symptom discussion, and recommendations

    • ‘Peace of Mind’ Check- $50 per check-in (30 mins)

      • Status review, medication adherence, overall client safety

    • Chronic Disease Management Session- $150 for initial session and $60 for follow-up visits (60 mins)

      • Personalized support for managing conditions like COPD, diabetes, hypertension, heart disease, arthritis, dementia/Alzheimers, and autoimmune diseases. Includes care plan coaching.

    • Essentials Package- $160/month

      • Includes: 1 Wellness Visit, 2 “Peace of Mind” Check-In, direct communication with providers as needed, appointment accompaniment, and monthly family meeting

    • Continuity Package- $270/month

      • Includes:  Everything in the essentials package in addition to 1 Medication Management session and priority scheduling.  

    • Chronic Care Package- $350/month

      • Includes: Everything in continuity package in addition to care plan coaching, provider communication and coordination, lab/testing follow-up 

    • Advocate Plus- $500/month

      • Fully customizable monthly package based on client needs. 

        • Includes: 

          • 2 ‘Peace of Mind’ check-ins 

          • Provider communication and coordination 

          • Biweekly family updates via Zoom, phone, or in-person.

          • Appointment preparation and follow-up

          • Lab/testing result review

    • Private Health Education- $75/session

    • Group Workshop- $400/group

      • Up to 15 people per group
