      • Supportive wellness visit that includes vital signs, status review, symptom discussion, and recommendations

    • ‘Peace of Mind’ Check- 30 minutes

      • Status review, medication adherence, overall client safety

    • Appointment Accompaniment

      • We DO NOT provide transportation.

  • Membership plans include a defined level of monthly coordination support. Additional time beyond the included scope is billed at the standard hourly rate of $95/hour.

    • Essentials Package- $195 per month

      ✔️ Medication review & management support

    • Chronic Care Package- $695 per month
